How do we ensure a coherent cross-sector healthcare system?
The Danish health sector is facing significant pressure over the coming years owing to:
- shortage of healthcare professionals
- an increasing elderly population with rising needs for care
- lack of digital transparency, collaboration and communication between sectors
- restructuring of the Danish healthcare system.
Politicians and healthcare management teams must therefore find solutions that optimise patient flow and create a coherent healthcare system.
This includes ensuring digital cohesion, so relevant health data can be shared across sectors – for the good of citizens.
Columna Axon gives professionals an overview of a citizen’s current situation via electronic care and health records, with information tailored to the role of the particular employee.
They are shown only relevant and up-to-date information from the past few days, making it quick and easy to access critical and relevant data. This is especially important when citizens transfer between healthcare players.
Columna Axon facilitates cross-sector sharing of data
Experience shows that the most effective solutions for the healthcare system have been developed through collaboration between public and private stakeholders.
With this in mind, Systematic designed Columna Axon to build digital bridges and create connections between sectors and stakeholders throughout the Danish health sector. This means that sectors can share the latest health data about the same citizen with one another.
Linking existing IT solutions
In Denmark, we have been sharing health data across sectors since 2013 using the Danish eHealth portal ‘Sundhedsjournalen’, and this is regarded by many as our common Danish record system.
Columna Axon will not change this, but will link together existing IT solutions, such as electronic health records (EHR) and electronic care records (ECR), thus facilitating sharing and collection of the most recent and relevant health data about citizens.
Up-to-date and relevant data
In other words, Columna Axon is a supplementary tool that supports transfers between sectors and promotes collaboration among healthcare professionals.
Real-time insight into a citizen’s treatment and care record will enable more focused efforts, as the various healthcare staff can gain a complete and up-to-date overview of the citizen’s situation. This could, for example, include the most recent treatment plan, blood test results, examinations or rehabilitation plan.
